What to bring to your first visit
- Photo ID and your insurance card.
- Completed new-patient forms, download from the Patient Forms page.
- A list of medications and supplements you take.
- Any dental imaging from a previous office (USB or have them email us).
- For emergencies: come in even if forms aren't ready. We'll handle it after you're comfortable.
Insurance and financing
We accept Delta Dental, Cigna, Aetna, BlueCross BlueShield, MetLife, United Healthcare, and most major PPO plans. CareCredit financing is available for treatment beyond what insurance covers, plus in-house payment plans for larger cases.
